Minimum Requirements:

High School diploma/GED with three to five years of institutional food service management experience. A combination
of education, experience, and training may be substituted when competency in the position is demonstrated. Must be
willing to obtain federally required food service management education within one year of hire date.

Specific Tasks:

Responsible for the day-to-day activities of the food operation. Plan, organize, and control the
workflow to assure an efficient operation. Assure the policies and procedures in the Food
Service Operation are implemented. Follow all relevant federal/state requirements for the food
service department. Provide leadership, support and guidance to staff to ensure that food
quality standards, inventory levels, food safety guidelines, and customer service expectations
are met. Create and maintain the food service department staff schedule. Train and evaluate
all dietary staff. Must be able to communicate effectively with all staff members, residents,
vendors, and community members. Must be able to perform the essential job functions of the
dietary aide and cook positions. May need to assist as necessary when other staff members
are absent. Maintain records of income and expenditures, food, supplies, personnel and
equipment and provide reports to the Administrator. Responsible for yearly inventory,
inventory report, monthly expense report, billing, and placement of monthly temperature
sheets. Work closely with dietitian in developing menu formulation regarding regular and
therapeutic diets as prescribed by physician orders. Maintain and update all dietary lists and
tray cards. Maintain appropriate continuing education to assure up to date credentials. Attend
resident care plan meetings, QAPI and other facility meetings. Other duties as assigned.

Updates for those Visiting Residents at Sansbury Care Center*

At this time, we are pleased to announce that visitation can resume immediately, both indoor and outdoor!  Outdoor visitation is preferable and strongly encouraged. 

OUTDOOR VISITATION:

We welcome your outdoor visits for all able residents. This is regardless of your vaccination status).

Residents who have tested positive for COVID-19 or who are in a 14-day quarantine period may not be able to receive guests outdoors.

INDOOR VISITATION:

We welcome your indoor visits at all times and for all residents (regardless of vaccination status), except for circumstances when visitation should be limited due to a high risk of COVID-19 transmission.

Limited visitation circumstances could include:

• Washington County’s positivity rate is greater than 10% AND less than 70% of our residents are vaccinated.
• Any resident with a confirmed COVID-19 positive test result or any resident that is in quarantine (whether vaccinated or unvaccinated) will not be able to receive visitors until they have meet the criteria for release from quarantine (14 days, no signs and/or symptoms).
• In the event that a resident and/or staff member tests positive for COVID-19, our facility would still implement all “outbreak mode” policies and procedures, which include:
  • ALL visitation (both indoor and outdoor) would be suspended until at least one round of facility-wide testing can be completed.
  • If there are no additional positive results at that time, visitation can resume.
  • If there is an additional positive result at that time, from within the same unit, then all visitation for residents (vaccinated and unvaccinated) in that unit will be suspended for at least 14 days while in “outbreak mode” but visitation may continue for all other units.
  • If there is an additional positive result at that time, from another unit, then all visitation for the facility would be suspended for at least 14 days while in “outbreak mode.”

REMINDERS:

• In the interest of all residents, we would like to request that NO visits be scheduled for Saturdays and Sundays past 2pm and not between the hours of 10:30am-11:30am due to Mass.
• All visits MUST be scheduled prior to visitation. Please contact Latia Spann, Activities Director, at 859-336-3974 ext. 6551.
• Visitations will be scheduled in 30-minute increments.
• ALL visitors MUST adhere to the screening process for signs and symptoms of COVID-19.
• Hand hygiene (use of alcohol-based hand rub is preferred) must be conducted.
• Face covering or mask (covering mouth and nose) must be worn in the facility at all times.
• Social distancing at least six feet between persons at all times must be maintained.
• There is limited movement within in the facility. When you arrive, please go directly to the designated area for the visit. Please do not walk around the halls or visit with other residents.
• Any visitor that does not adhere to all policies and procedures will be denied visitation.

Responding to the COVID-19 Virus

Sansbury Care Center understands that you may be concerned about the spread of COVID-19 and how it may affect our facility.  Ensuring that residents are safely cared for in a healthy environment is our greatest concern.  At this time we do not have any active cases in our facility.

As a result of national and state directives to all nursing homes, Sansbury Care Center is limiting resident visitations to prescheduled appointments and end-of-life care situations. Please contact our Life Enrichment Department, Latia Spann, at 859/481-6293 to schedule your visitation with your loved one and for more details.

In addition, Mass and Communion Services will not be open to the public until further notice.  We understand that connecting with family members and residents is important, therefore we urge you to consider alternate ways to contact them.  Some suggested ways to contact residents include by telephone, text, mail, email, Skype or Facebook.

As suggested by the Kentucky Department of Public Health, Sansbury Care Center has implemented health screening for all individuals that enter the facility.  Our center is also following the prevention steps recommended by the CDC and is actively monitoring all updates. Other agencies also being monitored for information are local and state health departments, the Centers for Medicaid and Medicare Services (CMS) and the Kentucky Office of the Inspector General (OIG).
